Telemedicine Q&A

Telemedicine is the use of telecommunications and information technology to provide clinical health care remotely.
Today, we have one physician per 325 patients in the US.
Medical schools provide about 18,938 new graduates who enter the workforce every year, according to the latest AMA statistics.
The US grows at a rate of 3.0 per 1,000 people per year (975k new patients every year).
As you can see, it will be impossible to keep up the pace of our growth as a country and still deliver good quality care that can reach everyone.
Telemedicine is the only viable solution in solving this problem. It will provide quality care while expanding our reach at all levels: outpatient and inpatient areas.
So, whether accepted or not, Telemedicine is the future of medicine, and it is here to stay.

Telemedicine applies to all sectors: treating direct to consumer, helping practices increase their reach to patients, monitoring chronic disease thereby preventing future hospitalizations and even treating patients in the hospital in an acute setting.
It could be as simple as a phone call or HIPAA-compliant video communication to your doctor to as sophisticated as being able to examine and listen to heart, abdominal and lung sounds remotely, allowing to start lifesaving treatment right away.

Benefits are endless; Here is a sampling:

1. A doctor (in private practice, nursing homes or ALFs) can monitor patients remotely and receive alerts when something is wrong, decreasing the possibility of a patient to go to the ER or being hospitalized.
2. The same doctor in practice can examine patients while away using a telemedicine teleconsultation portal back in the office, NH or ALF. This will give doctors more control of their time and quality of life while still delivering non-emergent care to their patients.
3. A patient can connect with a qualified board-certified physician from home via a HIPAA-compliant video conference to ask questions or even be directed how to perform a non-urgent medical exam from the comfort of home and can finally receive treatment in a timely fashion. No more long waits to get an appointment (american average time: 1-2 w eeks to see y our doctor), waiting (average american waiting times: 2 hours) to finally see your doctor (average time doctors spend with a patient: 7.5 minutes), and spending the whole day for just a flu or a cold that would have taken just minutes to take care of.
4. A patient is able, via a travel kit (equipped with peripherics), to go anywhere in the world and be examined, advised and eventually treated by her/his own primary care physician in the US.
5. Patients can overcome distance barriers and improve access to medical services that would often not be consistently available in distant rural communities.
6. It is also used to save lives in critical care and emergency situations.

Today we live in a unique era, more and more people are familiar with the use of technology and less afraid to use it.
In this current era, telemedicine has kept pace with technology, allowing us to do 90% of what we normally do on a face-to-face real physical encounter through telemedicine. The only thing we cannot do currently is “palpate a patient,” which is usually done by the medical aid (nurse, MA, PA, etc.) during the telemedicine encounter.

Definitely not, telemedicine is a very powerful tool to empower patients and increase the doctor’s reach. More than a replacement, I see it as a complement to medicine.
We probably will be able to do more each year as technology advances.
For example, the use of augmented and virtual reality might allow doctors to palpate patients remotely in the decades to come.

I do not think so.
We live in a pretty hectic society and, as we discussed earlier, more people are becoming increasingly familiar with technology and less afraid of it.
You have seen the numbers! It is impossible for the current amount of physicians in the workforce to reach every single patient. We need help and that help is coming in the shape of telemedicine and telehealth.
Patients today will face the reality of not being seen at all for several weeks or months while their condition worsens, or they can be seen by a highly qualified doctor in minutes via telemedicine in which the doctor can diagnose and implement treatment right away. What would you do?
There is a significant amount of evidence documenting the fact that telemedicine works and saves lives. The population will become more educated and understand that telemedicine is here to help!
People will also become more aware when it is beneficial to use telemedicine and of its limitations.

That’s a great question because, in fact, telemedicine is less expensive and more effective.
You are able to save time and money, and travel expenses. Doctors are happier because they can work from their offices, home or any part of the world.

Diet & Nutrition
Diet & <b> Nutrition </b>
Many of our patients already recognize that a diet of fast food and sugary sodas is a recipe for weight gain, diabetes and chronic health problems. But between the latest super-foods and trendy dietary supplements, it can be difficult to know what you should be eating. At SM Concierge Medicine, we recognize that each patient has unique wellness needs and no single diet is suitable for every patient. We work closely with each patient to help him or her achieve and maintain an ideal weight. We can also make recommendations for dietary supplements and other specialized nutrition needs.

While each patient is unique, our general recommendations are as follows:

  • Consume at least five servings of fruits and vegetables each day
  • Eat more lean protein, including seafood, and limit the intake of red meat
  • Swap refined sugars for whole grains
  • Limit the intake of sodium, saturated fats, trans fats, cholesterol, sugar and refined grains
Exercise
Exercise
Studies consistently show that exercise is one of the best ways to stay healthy and reduce your risk for heart disease, diabetes and other chronic health conditions. Depending on your health, we recommend at least 30 minutes of moderate exercise (such as walking), five days per week or 25 minutes of vigorous exercise (such as jogging), three days per week.
Smoking Cessation
Smoking <b> Cessation </b>
Smoking or using smokeless tobacco is one of the single greatest risk factors for heart disease and other chronic health conditions. No amount of smoking is safe, even “social smoking” with friends at a bar or exposure to second-hand smoke is dangerous. If you don’t smoke, congratulations for making this positive lifestyle decision. Whenever possible, avoid or limit your exposure to second-hand smoke. If you do smoke, we can provide smoking cessation assistance. Quitting now can reverse years and even decades of smoking-related damage to your body.

Frequently Answered Questions

What cost is associated with the comprehensive lab test?
As SM Concierge Medicine patient, you enjoy the convenience of having your lab work conducted here in the privacy of our office. This allows for an immediate discussion of your results as soon as they are available. As a patient, advanced blood testing is included. Any additional lab work is submitted to your insurance company for payment.
What is included in the 'comprehensive lab testing' that occurs during the first visit?
Our comprehensive lab work includes routine and advanced blood testing. These tests screen for various health conditions, including diabetes, metabolic syndrome, dyslipidemia, hepatitis, STDs, and more. Dr. Maghidman constantly updates our tests in accordance to current testing recommendations from leading medical societies and evidence-based medical groups. Our tests are independent of insurance companies and designed to give a complete picture of your physical health. A full, updated list of every test is available here: comprehensive lab testing.
What is the 'comprehensive physical examination' that occurs during the first visit?
You will get a head to toe physical examination by Dr. Maghidman, aiming to screen different types of disease.
Does every follow-up visit need to be in person?
No. We are sensitive to your schedule and recognize that it is not always possible to have follow-up visits in person. Only the initial consultation and the first follow-up visit with Dr. Maghidman must be an in-person visit. Subsequent follow-up visits may be conducted via email or over the phone, unless a medical condition requires an in-person visit.
How often will I need a follow-up visit?
Depending on your age, health and medical history, we may recommend a follow-up visit every three, six or twelve months. You are also welcome to schedule a visit with us at any time to discuss health concerns you may have.
What should I expect during each follow-up visit?
During each follow-up visit, we will monitor any health conditions you may have, discuss any health concerns, and provide recommendations for enhancing your overall well-being.
What should I expect during my first visit?
Our goal is to provide the highest quality care by building a personal relationship with each patient. In order to do so, we spend around two hours during the first visit taking your medical history, performing a comprehensive physical examination, and conducting comprehensive lab work, including advanced diagnostic blood work. This information helps us create a complete medical profile and understand your unique health and wellness needs.
If I am a SM Concierge Medicine patient, will I still need insurance?
Yes. You should still maintain your insurance, whether it is through a private insurance company or Medicare. You will need this insurance to cover the cost of hospitalization, visits to a medical specialist, radiology procedures, and any required lab work.
How do I become a SM Concierge Medicine patient?
To become a patient, please contact SM Concierge Medicine at 1-855-544-3258 and ask to speak with our office manager. She will assist you in becoming a patient of our medical practice.
I have a pre-existing medical condition; can I still become a SM Concierge Medicine patient?
Absolutely! At SM Concierge Medicine, we welcome all patients and place no restrictions on pre-existing medical conditions. Our only limitation is the size of our practice; if we are full, we will place your name on a waiting list for next-availability. We regret that we cannot accommodate all patients who wish to join.
Will my private insurance or Medicare cover my lab work?
Advanced blood testing is included for free once you become a SM Concierge Medicine patient. Should additional lab work be necessary, the lab company will directly bill your insurance company or Medicare on your behalf.
Do you accept private insurance or Medicare?
No, we only accept payment directly from our patients. In order to best serve our patients and provide a fully transparent relationship, we have severed all financial relationships with insurance companies and government agencies. You deserve the highest quality, proactive medical care that puts your needs as a patient first – focusing on wellness, prevention and disease management. Unfortunately, our health care system today is driven solely by the bottom line. Insurance companies now dictate what care a patient can or cannot receive. Physicians spend an average of ten minutes or less with each patient. In fact, Medicare has created incentives for doctors to see more patients in even shorter intervals. Physicians no longer build personal relationships with their patients, and health care is reactive rather than proactive. As a result, we waste billions every year managing preventable conditions, while the health of our patients is compromised. At SM Concierge Medicine, we believe there is a better way.

Advanced
Laboratory Testing

1. Micronutrient Lab Testing
Overwhelming scientific evidence confirms that vitamin deficiencies are associated with disease processes and the overall condition of one’s health. Vitamin, mineral and antioxidant deficiencies have been shown to suppress immune function and contribute to chronic degenerative processes such as arthritis, cancer, Alzheimer’s, cardiovascular disease and diabetes. Using advance genetic testing this test will tailor any of your vitamin, minerals and or antioxidants needs. Based on a revolutionary method that measures the biochemical function of vitamins, minerals, amino acids and antioxidants, the micronutrient Lab Testing from Spectra Cell’s Laboratories provides a powerful portrait of each patient’s long-term nutrient status.
2. Cardiometabolic Risk Assessment/Lipoprotein Particle Profile Test
Approximately 50 percent of people suffering from heart attacks have shown “normal” cholesterol numbers. Cholesterol testing has historically been used as the standard indicator for cardiovascular disease classified as HDL (good) or LDL (bad). However, it is actually the lipoprotein particles that carry the cholesterol throughout our body, not necessarily the cholesterol within them, that are responsible for key steps in plaque production and the resulting development of cardiovascular disease. Now there is an advanced cholesterol testing technology, which accurately measures both the density and number of lipoprotein particles. This test allows us, with much better accuracy to determine the Risk of Coronary Artery Disease in our patients.
